A broad ligament fibroid can also cause discomfort similar to that of pregnancy

Upon arriving at Alexis Hospital, a 42-year-old woman who appeared to be pregnant complained of significant discomfort and pressure in her lower abdomen. Upon examination by Dr Rajni Gupta, an Obstetrician and Gynaecologist at Alexis Hospital, a large abdominal mass was discovered, roughly the size of a mid-sized watermelon and resembling a full-term pregnancy. Further investigation via MRI scan revealed the mass to be a rare and highly vascular broad ligament fibroid.

Broad ligament fibroids are benign smooth muscle tumours that originate from either the broad ligament hormone-sensitive smooth muscle or secondarily from the uterine smooth muscle. The broad ligament is a layer of tissue that connects the sides of the uterus to the walls and floor of the pelvis. This tissue covers the uterus, ovaries, and fallopian tubes, as well as provides a connection for nerves and blood vessels to these organs.

Initially apprehensive about surgery, the patient was advised by Dr Rajni to undergo the procedure. With careful planning and preparation, the operation was a success, resulting in the removal of a large five-kilogram fibroid measuring 25 x 20 cm. Due to its vascularity and high risk of bleeding and injury to surrounding structures, the removal of the adherent fibroid was technically challenging. Despite this, the patient had a smooth recovery and was discharged on the third day after the surgery.

Dr. Rajni Gupta noted that while uterine fibroids are common, broad ligament fibroids of this size are very rare and make up less than 1% of cases. Upon her discharge, the patient expressed her surprise at the rarity of her case and her gratitude towards Dr Rajni Gupta and the multidisciplinary team at Alexis Hospital for the quality care she received.
